About the Role:
As a Non-Clinical Aged Care Assessment Officer, you will assess eligibility for Commonwealth Aged Care Services and support older clients to live independently.
Conduct non-clinical assessments for clients aged 65+ (or 50+ for Aboriginal or Torres Strait Islander people).
Develop and review support plans and work collaboratively with clients on their wellbeing goals.
Promote client choice and participation in care planning.
Use the Integrated Assessment Tool (IAT) to refer clients to appropriate services.
Meet individual and team KPIs to satisfy funding requirements.
Identify and manage risks, escalating when necessary.
Skills & Experience Essential:
Demonstrated understanding or experience in aged care or age care assessment.
Understanding of My Aged Care Gateway and the Commonwealth funded services for older people living at home.
Experience working within client reporting management systems.
Current Victorian driver’s licence, Working with Children Card and fully vaccinated against COVID-19.
