Circle of Hope Services (COHS) is a registered aged care provider delivering safe, high-quality in-home supports to older Australians aged 65 years and over who are eligible for government funding under the Australian Commonwealth Aged Care Support at Home program.

We Are Seeking
An experienced, passionate, and dedicated Aged Care Service Delivery Manager to join our team. This role provides an exciting opportunity to lead within a values-based organisation committed to equality, social inclusion, independence, and supporting older people to remain safely at home.
Our purpose is to deliver high-quality, person-centered, safe, and culturally responsive supports that align with the expectations of the reformed aged care system and uphold the rights, preferences, and dignity of older people.
About the Role
This is a predominately office-based role located in Greenacre, South-West Sydney, working Monday to Friday, 9:00 am to 5:00 pm.
The Service Delivery Manager will oversee COHS’s:
- Care Assessment, Planning and Review team
- Aged Care Portal, GPMS and system compliance
- Daily operational service delivery
- Alignment with the Aged Care Quality and Safety Commission’s regulatory requirements.
- Preparation for the Support at Home Program and transition arrangements.
Key responsibilities include:
- Leading intake, assessment, care planning, and scheduled reviews aligned with participant rights under the Aged Care Act 2024.
- Oversight of individualised care plans, consumer risk assessments, medication management, end-of-life care planning, and service rostering.
- Ensuring safe, quality, person-centered supports aligned with the Strengthened Quality Standards 2025.
- Building and maintaining stakeholder, consumer, and community relationships.
- Supporting business development and growth within the reformed aged care environment.
- Ensuring compliance with all relevant Aged Care legislation, rules, guidelines, and COHS policies and procedures.
The successful candidate will maintain strict privacy, confidentiality, and ethical practice in accordance with the Aged Care Act 2024 and COHS’s organisational Code of Conduct.
Selection Criteria Essential
- Tertiary qualification and significant experience within the NDIS, Health or Aged Care sector.
(Aged Care, Disability, Nursing, Mental Health, Social Work, or Business Management) - Minimum three (3) years’ office management experience with strong knowledge of the aged care legislative and regulatory framework, including:
- Aged Care Act 2024
- Aged Care Rules 2025
- Strengthened Quality Standards 2025
- Support at Home Program requirements and reform directions
- Demonstrated understanding of person-centered, trauma-informed, culturally inclusive, rights-based care.
- Ability to manage operational and administrative responsibilities within a KPI and compliance framework.
- High-level written and administrative skills, including the preparation of reports, correspondence, and documents using cloud-based systems.
- Compliance with COHS’s Records Management Policy and Procedures.
- Strong interpersonal, leadership, and communication skills, with the ability to support participants, carers, families, and relevant stakeholders.
- Willingness to complete the Aged Care ALIS eLearning Modules as part of induction.
- Current Australian Driver Licence and reliable vehicle
- National Police Check (completed within the past 90 days) or NDIS Worker Screen Check.
- Right to work in Australia
