About us:

Attainable Care is a leading Registered NDIS/TAC/WorkCover Provider dedicated to creating a world of possibilities for individuals with disabilities. We are dedicated to providing a range of tailored services, including Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) homes. We are currently seeking a passionate and committed Care Coordinator to join our team.

About the position:


Assess the needs of the NDIS participants and develop a personalized care plan.
Provide daily care and supervision, such as diet, living arrangements, personal hygiene, etc.
Ensure that the living environment of the participants is safe, comfortable, and clean.
Assist participants in following medical advice, and manage medications and other treatment plans.
Arrange for their participation in vocational training or learning support.
Provide emotional support and psychological counseling.
Develop and maintain strong relationships with participants, their families, care teams and other stakeholders to ensure effective communication and collaboration.
Monitor and report on participants' progress and any changes in their needs, ensuring timely and accurate documentation.
Implement and regularly review individualized support plans, adjusting strategies as required to promote participants' progress and well-being.
Conduct risk management and emergency handling, and take appropriate actions promptly.
Participate as required in staff meetings, supervision and training sessions.
Adhere to all relevant legislation, regulations, and organizational policies, maintaining the highest standards of professionalism and confidentiality.

Qualifications & Experience:


Ability to communicate and appropriately respond to the needs of people with disability.
Relevant working experience.
Highly developed written and verbal communication skills.
Strong interpersonal skills with the ability to communicate on all levels.
A high level of computer skills including Microsoft Office Packages.
Initiative, strong attention to detail and problem-solving skills.
Strong work ethic.
First Aid Certificate and CPR certification.
NDIS Worker Screening Check
Working with Children Check
Police Check.
Driver’s license.
