Your opportunity

Full Time - Servicing Clients Across the Western Sydney Area

As our Care Partner, you will ensure clients' needs and wants are met through effective, commercially sustainable case management, aligning with our strategy, quality standards, Vision, Mission, and Values.

By assessing clients, matching services and products to their needs, and optimizing budgets ethically, you will help us provide person-centred support that enhances wellbeing and independence.

With integrity and resilience, you will:

  • Ensure all clients are treated with empathy, dignity, and respect by managing services that give them choice in daily life, addressing physical, clinical, emotional, spiritual, and social needs
  • Assess current and potential clients, decide on assistance, and implement appropriate services, products, and assistive technology in line with needs and budgets
  • Maintain compliance with organisational policies, the Care Packages Program, Aged Care Standards, and legislative requirements through timely documentation and reporting
  • Build and enhance positive relationships with ACAT teams, health professionals, brokered providers, clients, and families to promote the service and organisation
  • Manage finances and budgets accurately, monitor safety in client homes, and contribute to team performance monitoring and service growth

What you'll bring

You are an empathetic, organised collaborator who inspires trust and excellence in client care. You bring strong case management skills and a commitment to our values of integrity, justice, compassion, and excellence, embedding them in everything you do.

You will also need:

  • Certificate IV in Aged Care/Community Services or equivalent (minimum)
  • Previous experience in Home Care/Community/Aged Care
  • Current driver's licence
  • Intermediate computer skills including MS Office

Ideally, you will also have:

  • Diploma in Case Management
  • Experience in client assessment, budget management, and relationship building in aged care
  • Knowledge of assistive technology and brokered services
