About the role:


As a Case Manager at MHCS, you will play a crucial role in supporting individuals with disabilities to navigate the NDIS system and access the services and supports they require. You will be responsible for assessing clients' needs, developing individualised care plans, coordinating services, advocating on behalf of clients, and monitoring their progress towards achieving their goals. About the Role:

Requirements and desirable qualifications:


You must have a current driver's license and own car to use on shifts.
Knowledge and experience working in NDIS administration.

Key Responsibilities as Case Manager:


Research, coordinate, and manage a range of supports to suit individual goals/needs across multiple providers.

Coordinate access to support items in an NDIS Participant’s Plan in line with the preferences of the participant to achieve the outcomes identified in the plan.

Ensure that service provision is informed by the participants’ individual goals/needs and is in line with current participant protocols or individual service plans, from Life Health Services or other services engaged through the Supports Coordination.

Work with the client and their natural supports to assess the quality and value of the supports provided and ensure that they are meeting the participants’ individual goals/needs.

Actively support participants to connect with their communities and encourage participation in both mainstream and community options.

Support individuals to build their capacity and networks with the aim of greater choice, control and independence in managing their personal lifestyle choices.

Participate in training programs as required, including specific training on individual participant needs.

Ensure completion of NDIS Participant Service Agreements and other appropriate documentation.

Documented evidence demonstrates that service procedures are followed.

Ensure all relevant policies and procedures are followed.

Work within the funding levels associated with each participant

Develop and record the progress of participant's identified goals to be achieved through their support.

Ensure that the appropriate resources are available to achieve the agreed service delivery and related tasks.

Work closely with external and internal stakeholders to achieve positive outcomes for the organisation.

Responsible for planning and managing your workload to ensure the delivery of quality services.

Represent MHCS at relevant stakeholder meetings.

Build and maintain relationships with stakeholders.

Participate in the Supports Coordination process for individual participants and monitor outcomes.

Filing the incident reports.

Perform other duties as requested, consistent with the classification level of the position and in line with the principle of multi-skilling.

Coordinate and support clients and workers arrange training and advice where necessary and advise management and report outcomes to NDIS.

Work with the participant and their support workers to assess the quality and value of the supports provided and ensure that they are meeting the participants’ individual goals/needs to achieve zero incidents.

Participate in training programs as required, including specific training on individual participant needs and self-development.

You will be a valued team member helping to provide services to support clients’ individual needs in both their home and community.

Training and support are provided from the start and throughout employment. You will be required to assist with after-hours and weekends on a rotating roster (if required).
