Role: Case Manager for Community Care
Type: Permanent Full-Time
Work days : Sunday-Thursday or Wednesday-Sunday (Must Be Willing to Work Weekends)
Location: Camberwell / Hybrid

About You:
Are you a motivated healthcare professional with experience in case management for community settings?
Do you excel in building client relationships and managing care plans with exceptional organisational, communication, and computer skills?
If yes, we want you on our team!
Your Role:
Assess initial intake details and interact with new and existing clients to identify service needs.
Create care plans that link clients with resources and support, helping them remain in their homes comfortably and with peace of mind.
Create and keep upto date client ADLS to ensure our caregivers are supported with information regarding the client during their shift.
Conduct client visits as needed, providing valuable insight and support to clients and their families.
Review and update care plans, goals, and services for existing clients, ensuring they have the support needed to continue living in their homes.
Escalate complex case needs as necessary.
Provide advice on service options and coordinate referrals.
Maintain accurate client records and ensure compliance with funding and legislative requirements.
Monitor service delivery and respond to changing client needs.
Ensure compliance with the Aged Care Quality Standards and relevant legislation.
Required Skills & Experience:
Minimum 2 yearsβ experience in Case Management for Community Care.
Tertiary qualification in Health or a related field (nursing preferred).
Capacity to work effectively both independently an as part of a multidisciplinary team, demonstrate accountability and willingness to take direction.
Possess a broad knowledge and experience of the Aged Care Quality and Safety Standards for Community Aged Care Packages.
Ability to work with urgency and initiative to prioritise and follow up tasks.
Excellent computer skills with min 60 WPM, organisational, and communication skills. Complete a WPM Typing test.
A National Criminal History Check/Police Check with no disclosable Court Outcomes or Convictions (no older than 3 years).
Updated vaccination status (COVID-19 and influenza).
A current, valid Australian Driverβs License.
