The Opportunity:
We are seeking Community Care Workers to join our supportive Client Connections team within the Community Care & Active Living department. As a Community Care Worker, you will deliver high-quality, person-centred support under the Commonwealth Home Support Program (CHSP) and the Home and Community Care Program for Younger People (HACC-PYP).
These programs aim to help people live safely and independently at home and in the community by:
Promoting independence and reablement – supporting clients to maintain and build daily living skills, rather than creating ongoing reliance.
Enhancing health, wellbeing, and social connections – enabling participation in community life and reducing social isolation.
Providing flexible, person-centred care – aligning services with each client’s goals, preferences, and cultural needs.
Supporting carers and families – ensuring sustainable care arrangements through respite and tailored assistance.
We offer both permanent part-time and casual opportunities, with shifts available Monday to Saturday between 7:00 am and 7:00 pm. Applicants must be available a minimum of four weekdays, starting as early as 7 am.
Key Responsibilities
Domestic Assistance (major component of role): Support clients with household tasks such as cleaning, laundry, mopping, vacuuming, shopping, and light meal preparation, ensuring a safe and comfortable living environment.
Personal Care: Assist with showering, bathing, dressing, grooming, medication prompting, and general hygiene needs, delivered with dignity and respect.
Individual Social Support: Provide companionship, assist with shopping, transport to essential appointments, and help clients remain connected to their community.
Respite Care: Provide in-home or community-based respite to support carers while ensuring clients remain safe and engaged.
Wellbeing Monitoring: Observe and report changes in clients’ health and wellbeing to support independence and safety.
Team Contribution: Collaborate with colleagues, attend staff meetings, and participate in training and development.
Administration: Maintain accurate and timely records to ensure quality service delivery and compliance.

To be successful in this role you will have the following:
A genuine commitment to person-centred care and supporting older people and younger people with disabilities.
Physically fit and able to perform everyday tasks such as vacuuming, lifting, and practical household support.
Knowledge of CHSP and HACC-PYP principles, including independence, wellbeing, social inclusion, and carer support.
Certificate III in Individual Support (or equivalent qualification) or relevant experience.
Current Level II First Aid Certificate & CPR.
Current valid Victorian Driver's licence and access to a safe and appropriately registered and comprehensively insured vehicle suitable for the purpose of transporting clients.
Must hold or be able to obtain an Employee Working with Children Check.
Available to work between Monday and Saturday, 7am and 7pm, weekends, public holidays, and irregular hours as service requires.
