Disability Service Coordinator

About the role

This is more than a coordination role, its an opportunity to make a real difference in people's lives while helping shape the future of a growing service.

As our Disability Service Coordinator, you will support the day to day delivery of services while leading a small, dedicated team of support workers. You'll build strong relationships with participants, their families and the community, ensuring supports are meaningful, consistent and person-centred.

You will also play a key role in helpins us grow by contributing to networking and building connections in the community.

What you'll be doing

• Developing and implementing individualised support plans for clients based on their unique needs and preferences
• Coordinating the delivery of a range of support services, including personal care, social and community engagement, and skills development
• Building strong, trusting relationships with clients, their families and other healthcare professionals to ensure seamless and integrated care
• Supporting client growth, goals and independence
• Engaging with Support Coordinators and other community stakeholders
• Contributing to marketing, networking and business growth
• Ensuring services align with NDIS standards and best practice
• Maintaining accurate records and preparing reports to comply with regulatory requirements
• Leading, mentoring and supporting a team of support workers to ensure the delivery of high-quality services

What we're looking for

• Relevant tertiary qualification in disability services, mental health or community services or a related field
• Experience in coordination, team leader or supervisory role, coordinating support services for individuals living with disabilities
• Strong leadership, communication and relationship building skills, with the ability to build trust and rapport with clients, families and staff
• Understanding of person-centred and recovery orientated practice
• An interest in growing yourself and the service and being part of the business journey
• Excellent organisational and problem-solving skills, with a focus on delivering personalised, client-centred support
• Knowledge of relevant legislation, policies, and best practices in the disability services sector
• A genuine passion for empowering individuals with mental health and promoting their independence and inclusion in the community
• Current Driver's Licence, NDIS Worker Screening Check and Blue Card (or willingness to obtain)
