About the Role:
We are seeking a dedicated and experienced Case Manager to join our Springvale team. You will be responsible for managing a portfolio of clients, assessing needs, coordinating services, maintaining compliance, and ensuring participants receive the highest standard of care tailored to their individual goals and preferences.
Key Responsibilities:
Provide professional, evidence-based case management aligned with participant needs and company policies.
Conduct assessments, develop strengths-based care plans, and coordinate implementation of services within individual budgets.
Collaborate with allied health professionals, GPs, and external services to address clinical needs.
Support clients through ongoing reviews, re-assessments, and service adjustments.
Ensure accurate scheduling of services, roster management, and staff communication.
Maintain participant documentation, care records, and ensure all data is up-to-date and compliant.
Manage participant daily activities, outings, emergencies, and critical incidents.
Coordinate birthday acknowledgements, health material purchases, and day centre programs.
Supervise support workers, monitor performance, manage timesheets, and coordinate training.
Deliver monthly statements, newsletters, and facilitate monthly team meetings.
Oversee infection control practices including COVID-19 safety protocols and staff PPE usage.
Assist the company manager with operational support and ad hoc responsibilities.
Qualifications and Skills:
Tertiary qualification in Social Work, Nursing, or a related community services field.
Minimum 3 years of case management experience, ideally in home care, aged care or disability services.
Understanding of person-centred care principles and individualised funding management.
Strong written and verbal communication skills with an ability to work across cultures and demographics.
Excellent time management and ability to prioritise in a fast-paced environment.
Proficient in Microsoft Office and experienced in using digital case management systems.
Demonstrated knowledge of relevant legislation and standards in aged/community/disability care.
Strong interpersonal skills to liaise with clients, families, care teams and external agencies.
Fluent in English, Chinese Mandarin and Cantonese.
Essential Requirements:
Driverβs license and own insured vehicle
Current First Aid Certificate
Willingness to undergo a Criminal History Check
Working with Children Check (desirable)
