We are seeking a proactive and organised Intake and Support Officer to join our dedicated team. This full-time role is integral to the smooth operation of our services, ensuring participants receive timely, coordinated, and compassionate support from the very first contact.
About the Role:
As the Intake and Support Officer, you'll be the key link between participants, support workers, and management.
Key Responsibilities:
Handle general enquiries and provide follow-up support to participants and families.
Coordinate onboarding and prepare associated documentation for new participants.
Manage rostering of Support Workers to ensure effective service delivery.
Identify recruitment needs and assist with screening of candidate resumes.
Conduct phone interviews and schedule face-to-face interviews.
Collate required documentation for onboarding and liaise with management for employment processing.
Review case notes and assist with incident reporting, ensuring information aligns with participant support plans.
Provide administrative and operational support across the organisation as required.
Develop and maintain person-centred support plans in collaboration with participants, their families, and key stakeholders.
Work collaboratively with participants, families, and staff to address concerns, identify root causes, and implement effective solutions.
Lead the implementation and oversight of staff induction and ongoing training programs to ensure alignment with participant needs, NDIS requirements, and internal policies.
Supervise staff performance, provide coaching and feedback, and promote a culture of accountability and growth.
You'll work closely with our Directors and the Development and Growth Manager to ensure seamless service delivery, maintain a positive team culture, and support the development of strong relationships within the disability community.
