About the Role

This is a leadership role within Myhorizon's Lifestyle Services, supporting participants across the Logan and Gold Coast regions.

As a Lifestyle Service Relationship Coordinator (SRC), you will provide practice-based leadership to Disability Support Professionals while overseeing service coordination, staff capability, and the safe delivery of services for participants with complex health requirements.

This role requires proven experience in High Intensity Daily Personal Activities (HIDPA) and the ability to lead staff practice in line with:

  • Participant support plans
  • Clinical and allied health guidance
  • Myhorizon policies and procedures
  • the NDIS Code of Conduct and Practice Standards

Key Responsibilities

Leadership, Training & Supervision

  • Provide direct leadership, supervision, coaching, and practice guidance to Disability Support Professionals
  • Ensure staff are appropriately trained, competent, and supported to deliver supports
  • Reinforce safe practice, escalation pathways, and professional boundaries
  • Contribute to onboarding, training plans, supervision, and performance processes

Service Coordination

  • Coordinate intake, transitions, and ongoing services for participants
  • Ensure support is delivered in accordance with risk assessments and support plans
  • Work closely with participants, nominees, and health professionals to maintain safe, person-centred services
  • Identify and respond to changes in participant needs, risks, or capacity

Quality, Compliance & Governance

  • Ensure services align with the NDIS Practice Standards, Code of Conduct, and Myhorizon policies and procedures
  • Participate in a rotating on-call roster
  • Complete accurate documentation, reporting, and data entry within required timeframes
  • Participate in audits, continuous improvement, and incident management processes

What You Bring

  • Proven experience supporting participants within their NDIS plan, including those with complex medical needs
  • Minimum 1 year of experience leading, supervising, and coaching staff
  • Confidence in applying organisational policies, procedures, and professional practice standards
  • Strong judgment, calm decision making, and the ability to identify and manage risk
  • Excellent communication skills with staff, participants, nominees, and external professionals

Required Qualifications, Skills & Checks

  • Diploma qualification in Individual Support, Community Services, Social Sciences or significant demonstrated experience
  • Experience supervising and leading staff
  • Sound knowledge of NDIS Practice Standards and the Code of Conduct
  • Intermediate computer skills (data entry, Microsoft Office/365)
  • Proven ability to provide clear written and verbal communication
  • Current Queensland Driver's Licence
  • Current First Aid & CPR (or ability to obtain prior to commencement)
  • NDIS Worker Screening Clearance (Yellow Card)
  • Queensland Blue Card
