About the Role

Our Head Office, based in Balcatta, is seeking a Full-Time NDIS Services Coordinator to provide high-quality coordination of services to people with disability and mental health diagnoses.

As a Services Coordinator, you will work closely with our team, NDIS participants, key stakeholders, and allied health therapists to develop, promote and maintain tailored supports for people with disability. You will be the central point of contact for participants, support workers, healthcare providers and stakeholders to flexibly coordinate supports and services to meet the holistic needs, choices and decisions of people with disability and their guardians or advocates.

Your Responsibilities:

The NDIS Services Coordinator will:

  • Advocate for participants’ rights, support their decision-making and address concerns.
  • Prioritise and coordinate services for a complex caseload.
  • Conduct regular visits to participants and their residences.
  • Undertake regular environmental assessments, audits, risk assessments, hazard reports and incident reports.
  • Complete Risk Assessments for each participant regularly and submit as required.
  • Assess and update medication management protocols for participants’ safety.
  • Identify training needs for assigned programs.
  • Monitor and adjust care plans to facilitate any change in circumstance.
  • Coordinate the intake of participants and their transition to services.
  • Implement, monitor and report on the use of Behaviour Support Plans according to the NDIS Practice Standards and NDIS Rules, including the use of restrictive practices.
  • Write and submit reportable incidents to the NDIS Commission. 
  • Monthly Reporting on the use of Regulated Restrictive Practices.
  • Calculate and monitor the use of participant funds.
  • Collaborate on financial management for participants.
  • Ensure participant documentation is regularly reviewed and updated.
  • Manage and coordinate a team of support workers.
  • Create and manage rosters and care schedules in collaboration with the Rostering Team, adhering to award guidelines and NDIS pricing arrangements. 
  • Engage in continuous quality improvement of systems and processes to ensure best practice, particularly in relation to the NDIS Practice Standards.
  • Have availability to work within a rotating weekend on-call roster.
  • Complete all necessary administration to the required standard, ensuring full compliance.


We are looking for someone who has strong experience in:

  • Writing and submitting reportable incidents to the NDIS Commission. 
  • Monthly Reporting on the use of Regulated Restrictive Practices.
  • Being a highly resourceful team player, comfortable working in a fast-paced environment, while remaining flexible, proactive, and efficient.
  • An in-depth understanding of the NDIS Practice Standards, Rules, Code of Conduct, frameworks and guidelines.
  • Familiarity with person-centred approaches and individualised support planning.
  • Demonstrating good judgment in a variety of situations and the ability to maintain a realistic balance among multiple priorities.
  • Developing and implementing continuous improvement systems and processes in participant service delivery in accordance with NDIS Practice Standards.
  • Report writing and maintaining participant records, including Service Agreements, care plans, risk management, complaints management, and incident management.
  • The delivery of Supported Independent Living and other NDIS-funded living arrangements.
  • Interpreting NDIS Plans and funding arrangements to maximise supports and services.
  • Understanding the Positive Behaviour Support Framework, including the implementation of Behaviour Support Plans, restrictive practices, and reporting requirements.
  • Rostering in accordance with the SCHADS Award.
  • Leading a team beyond supervising - displaying real leadership skills to support, engage and inspire staff.
  • Collating data for funding. 
  • Ability to build rapport easily with others and maintain professional relationships.

Additionally, you will need:

  • High level of computer skills and demonstrate the ability to learn new systems easily.
  • Proficient in both written and spoken English.
  • Working With Children Check.
  • NDIS Worker Screening Clearance.
  • Current valid Western Australian Driver’s Licence.
  • Minimum 2 years experience as an NDIS Service Coordinator. 
  • Certificate IV in Community Services (Desirable) 
  • Experience using PRODA.
  • NDIS Worker Orientation Module - ‘Quality, Safety and You’.
  • NDIS Supporting Effective Communication Module.
  • New Worker NDIS Induction Module.

Upon success, you will need:

  • Public Sector Aboriginal and Torres Strait Islander Awareness Training.
  • Mandatory Reporting Training for Child Sexual Abuse.
  • Trauma-Informed Practice Training.
  • Incident Reporting and Management Training.
  • Medication Management Training.
  • Infection Control Training for Disability Support Workers.
  • Professional Boundaries Training.
