SUPPORT COORDINATOR POSITION DESCRIPTION
POSITION PURPOSE:
To provide support to participants and their families to navigate the NDIS and assist them to achieve their goals by identifying and connecting participants with appropriate support services, as well as monitoring and reviewing their progress to ensure they are receiving the best possible care.
PERFORMANCE OBJECTIVES:
The primary responsibility of a Support Coordinator is to establish positive collaborative relationships with participants, assisting them to develop their plan, implement supports and participate more fully in the community. As a support coordinator, you will be responsible for ensuring that each participant’s plan is implemented, and their goals are achieved.
Key Responsibilities:
· Meeting with participants regularly both in person and over the phone to discuss their needs and goals
· Maintaining accurate records of participant interactions
· Ensuring participants received timely and appropriate services
· Providing education and training to participants and their families to help them better understand the NDIS
· Complying with NDIS code of conduct
Detailed Responsibilities of the Role:.
· Assess the needs of participants and complete individual planning documents for support coordination services (completed within 2 weeks of signing the service agreement), ensuring all documentation is explained to the participant and their support network.
· Monitor and updated individual planning documents to reflect the needs and changing circumstances of participants, all individual planning documents must be updated at a minimum yearly.
· Coordinate services with other providers such as healthcare professionals and community organisations
· Monitor the progress and outcomes of participants and review their support plans
· Complete vulnerability check-ins with participants
· Advocate for participants and their families to ensure they receive the best possible care
· Provide education and training to participants and their families to help them better understand the NDIS and their support options
· Provide options and link the participant to services to maximise opportunity and independence.
· Support the participant to develop, achieve and evaluate goals.
· Maintain high-quality record keeping and reporting.
· Develop and maintain formal and informal networks within the local community for ease of referrals and access to appropriate services and other supports to further participant’s achievement of goals.
· Monitor budgets and use of funds
· Submit timely reports to the NDIA and support the participant to prepare for review
· Monitor and manage conflicts of interest following the BHC conflict of interest management processes
· Support the participants informal supports to be involved in helping the participant connect with services
· Ensure required recommendations and quotes are complete
· Report on how funding for Support Coordination has helped to build a participant’s capacity, however incrementally. Justify how the support has been delivered and make any recommendations for future support needs.
· Assist participants to manage challenges in their own support environment and ensure consistent delivery of service
· Assist participants to:
o Understand their plan, the price guide and how budgets can be used
o Keep up to date with NDIS information, policy changes and ways of working
o Use the myGov and myplace participant portal
o Liaise with providers, explaining their unique goals, needs and preferences
o Understand and consider Service Agreements, ensuring maximum value for money
o Understand and complete accurate service bookings
o Resolve points of crisis
· Connect participants with advocacy services if required
· Report weekly to the Director
Screening Requirements:
· 100 points of ID.
· Right to Work in Australia.
· NDIS Worker Screening Check.
· An international police check (where applicable).
· Working with Children Check.
Key qualifications and skills:
· NDIS Worker Orientation Module Certificate.
· Infection Control Training Certificate.
· Relevant degree in disability, mental health, or aged care (desired).
· Demonstrated experience in support coordinator or case management.
· Thorough understanding of the NDIS Pricing Arrangements and Limits.
· Local knowledge of mainstream and funded community organisations
· Demonstrated experience of the NDIS and in-depth knowledge of the NDIS Pricing Arrangements and Limits, and NDIS Practice Standards and Rules
