About the Role

We are seeking a dedicated NDIS Support Coordinator to join our Families and Children’s Specialist Services team. This role supports NDIS participants with complex and multiple needs to navigate the NDIS, implement their plans, build capacity, and work towards meaningful, self‑directed goals.

You will work closely with participants, families, carers and a wide range of community and funded service providers to coordinate effective, person‑centred supports that promote choice, control, inclusion and independence.

This position is a fixed term 12-month contract that is part‑time (45.6 hours per fortnight), worked Monday to Friday between 8:30am and 5:30pm.

Key Responsibilities

  • Provide high‑quality, person‑centred NDIS Support Coordination services
  • Assist participants to implement, monitor and review their NDIS plans
  • Build participants’ capacity to understand the NDIS, exercise their rights, and maintain support relationships
  • Coordinate complex, multi‑disciplinary services and supports
  • Develop goals and progress reports that are measurable, evidence‑based and participant‑led
  • Advocate for participants and support them to provide feedback or raise concerns
  • Maintain accurate and timely documentation in line with NDIS and organisational requirements
  • Work collaboratively with families, carers, service providers, funders and internal teams
  • Operate within the NDIS Quality and Safeguards Framework and Junction’s policies and values

About You

You will bring:

  • A relevant qualification in Disability, Community Services, Social Work, or significant relevant experience
  • Demonstrated experience working within the NDIS environment
  • Sound knowledge of disability services legislation and practice frameworks
  • Strong communication, negotiation and relationship‑building skills
  • The ability to manage a diverse caseload and respond effectively in complex or crisis situations
  • A strengths‑based, trauma‑informed and culturally responsive approach
  • Well‑developed time management and organisational skills
  • NDIS Worker Screening Check
  • Working with Children Check
  • Drivers Licence
  • Willingness to have a police check completed if successful for the role
