Stellar Disability Supports is seeking a new team member to provide NDIS Support Coordination in Shepparton, Victoria.

As a NDIS Support Coordinator, we believe the key to your role is to provide a capacity building service to participants in receipt of a NDIS plan. This service should be client centred, driven by the wishes of the participant, and focused on seeing the participant live as inclusively within their community as they wish to whilst working towards their plan goals.

This requires taking the time and effort to really get to know your participants and working with them to implement their plan and connect to appropriate supports.

Doing this well should see you inform the participant of the tangible ways these supports must relate to their disability, the reasoning behind some of the limitations to NDIS, and connecting the participant to mainstream and informal supports alongside their NDIS supports.

What you'll be doing

  1. Develop and coordinate individualised support for NDIS participants to help them access relevant services and supports
  2. Liaise with NDIS, government agencies, and service providers to ensure participants receive the appropriate level of care and assistance
  3. Advocate on behalf of participants to ensure their needs are met and their NDIS funding is used effectively
  4. Advocating on behalf of NDIS to ensure the scheme becomes sustainable, spreading the current cost across all government care systems
  5. Monitor and review participants' plan and supports regularly, making adjustments as needed
  6. Provide guidance and information to participants and their families about the NDIS and available support services
  7. Maintain detailed case notes and records to meet NDIS reporting requirements
  8. Maintain regular and consistent communication with your participants and their care teams

 

What we're looking for

  1. Ideally you will have some experience in a similar NDIS support coordination or disability support role, though our primary wish is to employ somebody who is kind, hardworking, ethical, dedicated, and above all else, committed to the work we do and to this business.
  2. Strong knowledge of the NDIS and its processes, as well as the disability support sector. Again, this is an ideal, not a mandatory requirement.
  3. Excellent communication and interpersonal skills, with the ability to build trusting relationships with co-workers and participants.
  4. Problem-solving skills and the ability to think creatively to overcome challenges.
  5. Strong organisational and time management skills, with the ability to prioritise and multitask. This is the most crucial administrative task within the role.
  6. Tertiary qualification in a relevant or comparable field such as social work, occupational therapy, or community services.
  7. Current driver's licence and access to a reliable vehicle.
