We are seeking a motivated and compassionate Support Coordinator to join our team on a part time (0.5 FTE), 3 months fixed term contract basis. In this role, you will help us coordinate client NDIS plans, negotiate appropriate support, and access a range of mainstream services and community support.

About the Role - Key duties will include:

  • Manage a caseload of participants, ranging in complexity, to provide Support Coordination based on a person-centred approach
  • Obtain, record and manage service agreements and consent for each participant
  • Develop a Support Plan, Crisis & Barrier Plan and, or Complexity and Risk Plan if identified in the Complexity and Risk Assessment
  • Ensure that all participants client files and other documentation is up to date, complete and in line NDIS Practice Standards
  • Coordinate participantsโ€™ supports including informal, mainstream, community and funded
  • Build and develop high quality professional relationships with both external and internal stakeholders including participants, families, NDIS and ONCALL staff and teams

About you -  As the successful candidate, you will bring:

  • A sector relevant qualification, e.g. Bachelor of Social Work/ Psychology, Allied Health, Health Sciences or Community Services
  • A track record of supporting participants to access the right supports, to develop and achieve sustainable goals in line with their NDIS plan
  • The expertise to build the capacity of clients and their networks to coordinate their own supports
  • Your excellent assessment and planning skills that enhance outcomes for people with NDIS funded support
  • Strong administration and communication skills
