You will be responsible for assisting NDIS Participants understand and use their NDIS Plan to achieve their goals. As the first point of contact, you will work alongside participants to connect them with the right supports and services, build their confidence to manager their supports, and make informed decisions.

What you will do 

  • Build strong relationships with participants, families, providers and community services to ensure supports are coordinated, person centred and aligned with the participants goals and preference. 
  • Support participants to establish their MyID account, navigate the NDIS portal, negotiate services to be provided and support participants to establish the service agreements with providers of their choice.  
  • Support NDIS participants to make informed choices and ensure their rights are respected.  
  • Conduct thorough assessments of participant’s’ needs, strengths, and support requirements to develop comprehensive support plans that align with their NDIS goals.  
  • Link participants to mainstream and/or community services (e.g. housing, education, transport, health) as required.  
  • Build and maintain a robust network of service providers, ensuring the availability of diverse options to meet clients' requirements.  
  • Regularly review participants' NDIS plans, track funding and goal progress, and make necessary adjustments to ensure plan goals are met effectively.  
  • Maintain accurate and up-to-date records, including progress notes, service agreements, risk profiles, support plans as required. 
  • Assist participants to monitor and track their NDIS budgets, ensuring funds are utilised effectively and aligned with their individual goals.  
  • Stay informed about changes and updates within the NDIS scheme, disability services, and related regulations to provide participants with the most current and relevant information.  

What you will bring 

To excel

  • Relevant Tertiary qualifications with relevant experience or less formal qualifications (relevant Certificate 3 or above as identified by Afford) with extensive knowledge and demonstrated experience in the Disability sector or Human Services.   
  • Previous case management experience working with clients with complex needs, and a strong knowledge of the NDIS, and NDIS packages.  
  • Understanding of the NDIS price guide structure and flexibility within budgets, NDIS legislation and rules including provisions relating to reasonable and necessary supports.  
  • Previous experience working within the Disability or Human Services Sector 
  • Strong written and verbal communication skills, particularly in preparing reports and presenting performance data
  • Strong personal commitment to working in a for purpose organisation
  • Demonstrated ability to deliver high quality customer focussed services.
  • Experience managing customer complaints in a complex environment
  • Current ‘C’ class driver’s licence and access to a personal vehicle for travel to various Afford locations in NSW, with the ability to undertake regular interstate travel.
