NDIS Support Coordinator | Clear Vision Support Solutions

Clear Vision Support Solutions is a growing, values-driven organisation committed to delivering high-quality, person-centred support to NDIS participants across Australia. We are currently seeking an experienced and passionate Support Coordinator to join our team.

This is an exciting opportunity for someone who is organised, proactive, and committed to helping participants navigate the NDIS and achieve meaningful outcomes. You will work with participants located across Australia, supporting a diverse caseload with a strong focus on responsiveness, coordination, advocacy, and relationship building.


About the Role

As a Support Coordinator, you will support NDIS participants to understand and implement their plans, connect with appropriate services, and build their capacity to manage supports over time. You will work with participants, families, providers, and stakeholders across Australia to ensure supports are practical, effective, and aligned with participant goals.

This role requires someone who can support participants who do not live in their local region, engage with stakeholders and build relationships with new providers in participants areas, manage competing priorities, think critically, communicate effectively, and maintain a high standard of documentation and compliance.


Key Responsibilities

  • Support participants to understand and implement their NDIS plans
  • Coordinate supports and services for participants across Australia
  • Liaise with providers, families, allied health professionals, and other stakeholders
  • Assist participants to build capacity, exercise choice and control, and work towards their goals
  • Identify service gaps and support participants to connect with suitable providers
  • Monitor budgets and supports and address barriers impacting service delivery
  • Prepare for plan reviews and provide supporting evidence where required
  • Maintain accurate, high-quality case notes, records, and reporting
  • Manage risk, escalate concerns appropriately, and advocate for participant needs

About You

  • Experience in NDIS Support Coordination is preferred
  • Sound understanding of the NDIS, participant safeguarding, and person-centred practice
  • Strong administrative, communication, and time management skills
  • Ability to manage a national caseload with professionalism and attention to detail
  • Confidence working independently while remaining part of a collaborative team
  • Strong problem-solving skills and ability to build rapport with a wide range of people
  • Genuine commitment to achieving positive outcomes for people with disability

Essential Requirements

  • Current Driver’s Licence
  • Reliable vehicle
  • Current NDIS Worker Screening Check, or willingness to obtain
  • Current Working With Children Check/Blue Card
  • National Police Check
  • Relevant qualifications in Community Services, Social Work, Human Services, Disability, or similar are desirable
