AmeCare is a highly dedicated and specialised agency supporting people with mental heath and dual disabilities.
We are pleased to announce that due to a rapid growth, we are currently looking to recruit a full-time Support Coordinator to join our team.
Roles and Responsibilities:
- Provide support to participants with mental health or disability to live a full and contributing life.
- Assist participants to take more control of their lives and to better manage complex challenges of day to day living.
- Through strong and respectful relationships and skilled coaching, participants will be supported to build capacity, including strengths and resilience.
- Work collaboratively with participants, their families, carers and supports to design, plan, implement and adjust a recovery plan.
- You will collaborate with the broader service system and will assist with the coordination of NDIS and other supports.
- Service Agreements are in place for all Participants
- Work with participants to achieve their goals in line with their NDIS plan.
- Maintaining appropriate and professional boundaries
- All supports are delivered, invoiced and recorded accurately in our client management system
- Information on relevant community resources are made available to clients and referrals are made and followed up on
- Your caseload/tasks are managed in a coordinated, efficient, and timely manner.
- All participants are linked with service options corresponding with their NDIS Plan support items and funding.
- A range of options are provided to participants to select from to achieve the outcomes in their NDIS Plan
- Maintain a working log of all contacts with clients, families/carers and support services within the CMS.
- Documented evidence demonstrates that organisational policies and service procedures are followed.
- Outstanding communication and liaison skills are demonstrated at all times, with internal and external stakeholders.
- Participant records are updated, reviewed, and kept in order.
- Participant issues are reported to management as soon as practicable.
- Procedures are understood and consistently followed with clarification sought when needed.
- Complying with all relevant legislation and codes of conduct.
- Provide Crisis support to participants where required.
- Assess and manage risk in line with organisational policies and procedures.
- Engage in a professional manner with all NDIA staff and LAC's.
- All reportable incidents must be reported to your manager as soon as possible following appropriate procedures.
To be eligible for the position:
- Be self-disciplined with effective time management and balancing your workload to achieve the agreed efficiency KPI of 80% billable time (based on your committed hours as agreed with your manager on a monthly and quarterly basis).
- Accurately manage the budget of your participant's NDIS Plan, providing transparency to participants.
- Actively demonstrate efforts made towards assisting participants to achieve their goals.
- Accurate and consistent case noting to be captured as you go or within 24 hours.
- Support participants to engage with both mainstream and NDIS funded services and programs.
- Provide access to activities and opportunities that promote independence.
- Adaptability β Adjust to changing circumstances and prioritise tasks.
- Client Advocacy - Commitment to social justice, inclusion and valuing difference and diversity.
- Integrity β Undertake all your actions to the highest level of personal integrity.
- Ability to collaborate with others to achieve common goals.
Requirements
- A degree in Social Work or Mental Health or equivalent training and/or a minimum two years of experience in mental health related work highly desirable
- Previous experience as a Support Coordinator highly regarded
- Specialist knowledge and skills in psychosocial recovery, mental health, and service navigation within the mental health system.
- Complex case management experience
- Current Driver's License, current Fully Comprehensive Care insurance and ability to undertake travel as required.
- Current First Aid and CPR
- Working with Childrens check
- Current NDIS Workers Screening & NDIS Worker Orientation Module
- NDIS Worker Orientation Module completion may be obtained after hiring if not already held.
- Knowledge of trauma informed care
