About the role
Bowen Flexi Care is seeking an experienced and dedicated NDIS Support Coordinator to join our team in Bowen, Queensland. This is a part time role that will involve providing Support Coordination to NDIS participants in our local community and beyond. You will play a pivotal part in helping our participants maintain their independence and quality of life.
What you'll be doing
- Coordinating and monitoring the delivery of in-home and community-based support services
- Liaising with participants, their families, and other healthcare professionals to ensure seamless care
- Advocating for participants and their needs, and connecting them with relevant community resources
- Maintaining detailed participant records and documentation in line with organisational and regulatory requirements
- Contributing to the continuous improvement of our support services
What we're looking for
- Relevant qualifications in community services, disability support, or a related field
- Demonstrated experience in a similar support coordination or case management role
- Strong communication and interpersonal skills, with the ability to build rapport with participants
- Excellent organisational and problem-solving abilities
- A genuine passion for supporting people with disabilities to live independently
- A current driver's licence and access to a reliable vehicle
