About the role
Horizon- Support Coordination and Disability Services is seeking an experienced NDIS Support Coordinator to join our team in Shepparton, VIC on a part-time basis. As an NDIS Support Coordinator, you will play a crucial role in empowering individuals with disabilities to access and coordinate the services and supports they need to live independently and achieve their personal goals.
What you'll be doing
- Developing and implementing comprehensive support plans to meet the unique needs and preferences of each client
- Coordinating and monitoring the delivery of services from a diverse range of providers, ensuring seamless integration and optimal outcomes
- Appropriately supporting clients to ensure they receive the necessary supports and have a strong voice in the decision-making process
- Regularly reviewing and updating support plans to reflect changing needs and goals
- Maintaining detailed case notes and documentation to comply with NDIS requirements
- Collaborating with other professionals, such as therapists and case managers, to provide holistic support
- Proactively identifying and addressing any barriers or challenges that may impact a client's ability to access services
What we're looking for
- Relevant tertiary qualifications in the community services, social work, or allied health fields
- Experience working as an NDIS Support Coordinator or in a similar role within NDIS
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, their families, and a wide range of service providers
- Strong problem-solving and case management skills, with the ability to navigate complex systems and advocate effectively on behalf of clients
- Demonstrated commitment to the principles of person-centred care and supporting individuals with disabilities to achieve their goals
- Proficient in using relevant software and documentation systems to maintain accurate records and reporting
- A valid driver's licence and the ability to travel within the local area as required
- Australian citizenship is required for this position
- Current Working with Children Check and NDIS Worker Screening Check are mandatory requirements for this role
