As part of our continued growth, we are seeking an experienced Support Coordinator to join our Western Australia team based in Malaga.
This is a 30-hour per week flexible part-time role, with the potential to transition to full-time as the caseload grows.
About the Role
You will work closely with NDIS participants to implement their plans, coordinate supports, and build their capacity to navigate the NDIS independently.
Key responsibilities include:
- Supporting participants to implement their NDIS plans in line with their goals, funding, and preferences
- Coordinating and connecting participants with appropriate providers, services, and community supports
- Monitoring service delivery, budgets, and outcomes to ensure supports remain effective and sustainable
- Maintaining accurate case notes and documentation in line with NDIS and organisational requirements
- Preparing progress reports and review documentation for reassessments and plan variations
- Liaising with families, carers, providers, NDIA representatives, and other stakeholders
- Identifying risks, service gaps, or safeguarding concerns and escalating appropriately
- Promoting participant capacity building, choice, and control in all decision-making
This role requires someone who is confident in managing a caseload independently, has strong attention to detail, and maintains professional boundaries and compliance standards.
About You
We are looking for a proactive, organised professional who is passionate about advocating for participants and improving outcomes.
Essential Requirements:
- Demonstrated experience working in the NDIS sector
- Strong understanding of NDIS processes, funding categories, and plan implementation
- Ability to manage a caseload autonomously
- Excellent written and verbal communication skills
- Current Driver’s Licence and reliable vehicle
- NDIS Worker Screening Check
- Working With Children Check
- Full working rights in Australia (Citizen, Permanent Resident, or valid visa with work rights)
Desirable:
- Qualification in Social Work, Community Services, Psychology, Allied Health, or related field
- Experience supporting participants with complex needs
- Experience in Local Area Coordination (LAC)
- Experience in Case Management
