About Somerville Community Services

Somerville is a for-purpose, community-based organisation that has supported Territorians for over 60 years. We specialise in high-quality disability services that empower people to live the life they choose. As a registered NDIS provider, we deliver individualised support across the Northern Territory, with a focus on respectful, person-centred relationships that promote choice, dignity, and independence.

About the role

We are seeking a motivated professional to join our team in Alice Springs as a Support Coordinator. This role requires either experience in support coordination or a strong background in disability, community services, or social work, along with a good working knowledge of the NDIS.

You will work closely with NDIS participants to help them understand their plans, coordinate supports, and connect with services that reflect their goals and aspirations. You’ll collaborate with families, service providers, and community organisations to ensure supports are well coordinated and responsive to participant needs.

In this role you’ll:

  • Support participants to understand and implement their NDIS plans
  • Coordinate services and supports that align with participant goals
  • Connect participants with appropriate service providers and community supports
  • Facilitate referrals, obtain quotes, and coordinate service agreements
  • Monitor and review supports to ensure they remain effective and responsive
  • Assist participants to prepare for NDIS plan reviews, including gathering reports and supporting documentation
  • Work collaboratively with families, guardians, providers, and government agencies to achieve positive outcomes
  • Maintain accurate case notes and documentation in line with NDIS and organisational requirements
  • Identify and respond to risks, barriers, or changes in participant needs

What you’ll bring

You will bring strong communication and coordination skills, along with a commitment to person-centred practice. To succeed in this role, you will demonstrate:

  • Experience working in disability, community services, social work, or a related human services field
  • Strong understanding of the NDIS, including plans, supports, and funding structures
  • Experience supporting people to navigate services and coordinate supports
  • Ability to build positive relationships with participants, families, guardians, and service providers
  • Strong organisational and time management skills
  • Ability to manage competing priorities and complex situations
  • Confidence using Microsoft Office and electronic client systems
  • Current (or ability to obtain) Ochre Card, Criminal History Check, NDIS Worker Screening, First Aid and CPR, NT Driver’s Licence and influenza vaccination

A qualification in disability, social work, community services, or a related field will be highly regarded.
