NDIS Support Coordinator Position for 'Inclusive Disability Assist'

Inclusive Disability Assist are a community focused not-for-profit organization that delivers high quality, person-centered support to NDIS participants. As part of the Marsden Families Program, our organization has existed for over 20 years.
Our core focus is to support participants to become empowered and identify which NDIS supports they require, and how they’d like them delivered. We treat a participant's NDIS goals very seriously and ensure that this is the focus throughout a participant's journey.
As a registered NDIS service provider, we specialize in delivering NDIS Support Coordination and Specialist Support Coordination to participant’s who have met access to the National Disability Insurance Scheme with a psycho-social disability. Our caseload is highly diverse including supporting not only individuals who have met access to the NDIS but also children and families.
Contract Level 2 Support Coordination Position Available
An IDA Support Coordinator works collaboratively with participants and their respective support networks to:
- Interpret and implement their NDIS Plan within the context of the NDIS framework, mainstream and community services.
- Manage multi-disciplinary teams and services in the coordination of a participant's NDIS funded supports, mainstream supports and informal supports.
- Identify appropriate supports, engage with new and existing support providers and work towards achieving identified outcomes and goals.
- Holistically address points of crisis, barriers and participant risks and design solutions to manage these risks.
- Build and sustain strong collaborative relationships with government and non-government bodies, service providers, allied health professionals and practitioners.
- Co-ordinate stakeholders to ensure a collaborative and sustainable approach across all areas of support.
- Build participant’s capacity to self-direct, self-advocate, work with professionals and other supports and manage their lives and their NDIS plan the way they choose.
- Adhere to thorough case-note recording, and prepare reports as required in line with the NDIS Support Coordination requirements.
About You
Essentials:
We are seeking applications from experienced, highly skilled and committed candidates who lead with a person-centred approach and can demonstrate the following key criteria:
- Experience working in a Support Coordination, Psycho-Social Recovery Coach or Case Management based role, identifying and linking participants to services that cover a diverse range of support needs.
- Excellent interpersonal skills that value individual and cultural diversity and which foster productive working relationships with a wide range of stakeholders.
- A strong solution focus and the ability to think outside of the box and be creative.
- Demonstrated ability to identify and provide a range of solutions to complex challenges, barriers and risks that face participants and work with stakeholders to manage such challenges.
- Demonstrated experience working collaboratively with a small team where you show excellent communication skills and a willingness for continuous learning.
- In-depth working knowledge of the National Disability Insurance Scheme (NDIS) and operational guidelines and experience in coordinating a range of community-based supports and methods to achieve participant goals.
Desirables:
- Allied Health or other relevant tertiary qualification is highly valued however not essential
- Working knowledge / skills / experience in complex mental health, complex health conditions and/or in Housing.
- Understanding of the National Disability Insurance Scheme framework
You will have or be willing to obtain:
- Current Queensland driver’s license and a well-maintained and fully insured vehicle that meets current safety standards.
- Current NDIS Worker Screening Clearance
- Current Blue Card (Working with Children Check)
