About the Role
In this role, you will:
- Support participants to understand and implement their NDIS plans
- Coordinate and connect participants with quality service providers
- Build participant capacity through a coaching and empowerment approach
- Monitor budgets and ensure supports are sustainable
- Maintain accurate, high-quality case notes and documentation
- Work collaboratively with families, providers, and NDIS stakeholders
- Identify and manage risks in line with best practice
- Stay up to date with NDIS legislation, processes, and local services
About You
You’re someone who is:
- Experienced in Support Coordination or the broader NDIS/disability sector
- Confident navigating NDIS plans, funding, and reporting requirements
- Highly organised with strong attention to detail
- A clear and compassionate communicator
- Able to build strong, professional relationships
- Solutions-focused and proactive in your approach
- Comfortable managing a flexible, largely autonomous workload
What You’ll Bring
- Experience in NDIS or a Registered Provider setting
- Strong understanding of NDIS legislation, frameworks, and documentation
- Excellent written skills (case notes, reports, communication)
- High level of professionalism, empathy, and accountability
- Ability to work both independently and as part of a team
Requirements
- Current driver’s licence and reliable vehicle (for participant visits as required)
- Right to work in Australia
- Willingness to complete required checks (NDIS Worker Screening, references, etc.)
