About the Role:

(Please note: while our office is based in Collingwood, this role covers caseloads in the Mornington Peninsula and Melbourne’s Eastern Suburbs.)

This is an independent contractor role, where you’ll manage your own business obligations (ABN, insurances, tax, super, etc.) while partnering with CoAbility to deliver high-quality Support Coordination.

Location and Travel Requirements:

This is a fully remote contractor role, with the flexibility to work from home. You will also need to travel locally for participant meetings and community visits within the Mornington Peninsula and Eastern Suburbs. Applicants must live within a reasonable commuting distance of this region.

The Opportunity:

By contracting with CoAbility, you’ll benefit from:

Supportive Leadership:
Access mentoring and supervision from experienced leaders.
Collaborative Team Culture:
Be part of a values-led, inclusive, remote team.
Flexibility:
Choose your hours and manage your own schedule.
Admin Support:
We handle referrals, intake, and onboarding, so you can focus on participants.
Established Reputation:
Partner with an independent provider trusted since 2017.
Competitive Contractor Rates:
Enjoy fair and transparent remuneration.

The Role:

As a CoAbility Support Coordinator, you will play a pivotal role in providing exceptional support services to Level 2 and 3 participants.

Your primary responsibilities will include:

Supporting participants to understand their NDIS plans and exercise choice and control.
Connecting participants with supports, negotiating service agreements, and removing barriers.
Assisting with plan reviews, reports, and achieving measurable outcomes.
Building trust and maintaining continuity when stepping into existing caseloads.
Collaborating with stakeholders to strengthen participant networks and resilience.
Upholding CoAbility’s values and person-centred practices.

What We’re Looking For:

Relevant tertiary qualification (e.g., Social Work, Community Services, Allied Health) and minimum 2 years’ experience in Support Coordination or case management.
Strong knowledge of the NDIS and disability support sector.
Excellent communication, organisational and problem-solving skills.
Ability to work independently while staying connected to a team
A genuine passion for making a difference in the lives of people with disability.

You will also need:

ABN, with capacity to contract as an independent provider.
Current Professional Indemnity and Public Liability Insurance.
Full Australian working rights.
Current NDIS Worker Screening Check.
Current Working with Children Check.
A valid Driver’s license and a comprehensively insured motor vehicle.
100 points of identification.
