The Role:


As a Program Coordinator at Morcare Services, you will draw on your experience in planning and coordinating services to ensure support is delivered to people in their own homes and within the community. Morcare Services is committed to service excellence and making a positive difference to our clients to enhance their quality of life.

The successful applicant will liaise closely with clients, their families, case managers, support workers, and allied health professionals to coordinate the provision of in-home supports in the Melbourne Metro region.

About You:


We are looking for an individual who has strong interpersonal skills, is a critical thinker, thrives on challenges and problem solving and has experience in assisting clients to coordinate their program activities and daily living supports.

Ideally, you will possess a Diploma or Certificate IV in Community Services or a Disability-related field, Registered or Enrolled Nursing, or Occupational Therapy.


Knowledge:


A strong understanding of the NDIS, TAC, and Worksafe principles, and how they operate, including funding and service agreements.
Experience in completion of client assessments and development of individual care plans.
Strong interpersonal, critical thinking, and problem-solving skills.
An ability to liaise with clients, case managers, treating health professionals, and staff to meet specified program goals and objectives.
An understanding of the needs of clients with a disability, their advocates, and family members.
High level of computer skills, including sound knowledge of Microsoft Office products.
The ability to complete tasks with minimal supervision and to deadlines.
Well organised planning and time management skills.
An ability to work in a team environment or individually as required.

Key responsibilities will include:


Managing a caseload of Clients with a disability including Clients for other coordinators as required.
Implementing strategies to respond to identified client care needs and goals ensuring enablement and personal choice.
Provide advice to Clients regarding the full range of services within the funding allocated and ensure Clients are provided with options to best utilize their funding package.
Document Lifestyle Support Plans, training checklists, hazard and clinical management guidelines ensuring that they are comprehensive, promote independence, reflect goal progression, and provide current detailed instructions eg: training/instruction checklists.
Arrange regular face-to-face Client contact to review and assess progress towards goals and update Lifestyle Support Plans, training checklists, hazard and clinical management guidelines at least on an annual basis or when there is a change to the program.
Ensure Support Workers are informed of Clients’ care needs.
Work with internal stakeholders to provide excellent client services.
Manage feedback from Clients and other Stakeholders, report to external funding bodies as legislated and escalate to Team Manager.
Responsible for recording and providing feedback in regard to Support Worker performance including discussing any concerns with the Support Worker involved.
Attend and participate in Team Meetings/Case Management/Planning/Functional Independence Review Meetings/Support Worker Training etc, as required, arrange for other employees to attend these meetings as required and communicate any rostering changes to Rostering team.
Identify any specific or unique Client needs that require specialised training or skills, report these needs to Team Manager - Disability for further consideration.
Contribute and participate in Support Worker annual reviews.

Essential Requirements:


Full working rights within Australia.
Current Level 2 First Aid certificate or willingness to obtain one.
NDIS Worker Screening Clearance or willingness to obtain one.
Certificate IV in Disability (or similar).
Australian Drivers Licence and your own vehicle as travelling to client's homes is vital to build relationships with our clients.
