About the Role:
We are seeking an experienced and passionate Program Manager to lead and oversee the delivery and coordination of high-quality aged care services across our In-Home Support programs. This is a key leadership role where you’ll ensure our clients receive safe, responsive, and person-centered care that enhances their independence and wellbeing.
You’ll lead a dedicated team of Care Managers and support staff, manage operational efficiency, and ensure compliance with Aged Care Quality Standards, Home Care Package (HCP) guidelines, and funding requirements.
Key Responsibilities:
Lead the operational delivery of In-Home Support services across multiple programs and locations.
Ensure care plans are person-centred, goal-oriented, and aligned with funding guidelines.
Manage client intake, onboarding, reassessments, and service plan reviews.
Oversee a team of Coordinators, Rostering Officers, and Support Staff.
Be responsible for primarily growing and developing the program.
Provide staff supervision, coaching, and professional development.
Drive service quality, performance improvement, and risk management.
Ensure compliance with Aged Care Quality Standards and internal policies.
Manage service agreements, care budgets, and reporting obligations.
Foster strong relationships with clients, families, service providers, and stakeholders.
About You:
Minimum 5 years of leadership experience in community care, aged care packages, or In-Home Support Services.
Strong knowledge of Aged Care Quality Standards, CHSP & HCP funding, case management, and care coordination.
Proven team leadership and people management skills.
Proficient with aged care portals and care management systems (e.g., My Aged Care).
Relevant tertiary qualification in Nursing, Social Work, Allied Health, Aged Care or related field.
Solid understanding of aged care funding, budgeting, and compliance.
Current Police Check, First Aid Certificate, and valid Driver’s Licence.
Strong computer literacy in CRM/CMS systems and Microsoft Office.
