Qualifications & experience

  • Certificate or Diploma qualification or equivalent in Care (Desired but not required).
  • NDIS Workers Screening Clearance (Or willing to obtain).
  • Working with Children Check (Or willing to obtain).
  • Criminal Record Check (Or willing to obtain).
  • Experience in Case Management and/or relevant Case Management Qualifications (Desired but not required).

Β Tasks & responsibilities

  • Preparing and transmitting schedules for customer services.
  • Rostering - Arranging replacement staff to cover planned or unplanned absences and shift changes.
  • Stake Holder Management - Take ownership for developing and retaining/maintain these networks.
  • Taking of enquires in relation to current and potential NDIS participants.
  • Assist and support a team of Disability Support Workers.
  • Contribute to the development, implementation, monitoring and review of care plans to meet clients care needs and to promote quality of life.
  • Promote a consultative and collaborative approach to client care planning and quality managed activities.
  • Carry out comprehensive and accurate client assessments.
  • Maintain appropriate records and documentation. Complete all necessary administration to the required standard, ensuring full compliance
  • Fulfil other duties as required by management and other department personnel as requested/required. After-hours and on-call duties will be required, the appropriate award and standby rates will apply.
  • Occasional client transport.
