About the Role:
As a Service Coordinator at Meraki Care, you will be responsible for overseeing the day-to-day coordination of participant supports across our Supported Independent Living (SIL) homes and community services.
This role involves working closely with participants, families, staff, and other stakeholders to ensure high-quality, person-centered support and smooth service delivery.
You’ll take the lead in managing staff teams, maintaining compliance and documentation, and ensuring each participant’s goals and needs are consistently met.
Key Responsibilities
- Coordinate participant onboarding and ensure a smooth transition into Meraki Care services.
- Liaise with participants, families, and external stakeholders to foster strong, collaborative relationships.
- Conduct regular house visits to monitor participant wellbeing and ensure the smooth running of SIL houses.
- Oversee staff management and team performance, providing support, guidance, and supervision.
- Manage rosters for small teams, including managing daily shift changes and coverage.
- Maintain accurate documentation and daily records for each participant in line with NDIS requirements.
- Organize and coordinate participant appointments, community access, and day-to-day activities.
- Prepare, review, and update participant reports and support plans.
- Ensure the efficient, safe, and person-centered operation of SIL houses and services.
- Uphold compliance with NDIS Quality & Safeguards standards and Meraki Care policies.
About You
- Previous experience as a Service Coordinator, Team Leader, or Support Coordinator in the disability sector (preferred).
- Excellent communication, leadership, and problem-solving skills.
- Strong organisational and time management abilities.
- High attention to detail and a commitment to maintaining accurate records.
- Experience using rostering and client management systems.
- Understanding of NDIS guidelines, SIL operations, and person-centered practice.
