Most of your work will be working from home and a manageable caseload within a defined catchment area. You will have access to relevant professional development courses as identified to support your learning needs.

Continued training and professional development to support the NDIS pathway. In-depth orientation and ongoing training and support provided as required face to face, video link, phone, emails etc

ABOUT THE POSITION


We are seeking a highly motivated Social Workers and Support Coordinators who will work with a range of NDIS clients either part time or full time to assist hospital discharges to the community or assist new NDIS clients access the community. You will make a real difference in the quality of participants' lives by applying evidence-based practice to your work. You will work with a range of clients who have neurological and progressive conditions, physical and sensory impairments, intellectual disabilities and psychosocial conditions.

You will work from a person-centred approach and conduct a range of assessments and interventions in line with NDIS funding, NDIS rules and the Reasonable and Necessary Guidelines. You will have the ability to work autonomously and manage your own caseload. You will work with organisational IT systems to record client information whilst protecting privacy and confidentiality.

ABOUT YOU


A Social Work Degree (Bachelor or Masters)
Allied Health Degree - we are happy to work with allied health professionals who are seeking to work in the Support Coordination space
Diploma in Disability Services or other Disability Related professions
Knowledge and understanding of the NDIS framework and the disability sector is desirable
Ability to conduct comprehensive psycho social assessments, identify needs and work towards client goals
Desirable with writing NDIS reports for the purposes of plan reviews, exploring housing options and other NDIS processes
Ability to work independently and unsupervised within hours and targets negotiated
Ability to work face to face with clients and over telehealth
Proficient in the use of information technology
Your own car with current driver’s license, home office set up
Successful candidates are required to undertake a Police Check, Working with Children Check and NDIS Worker Screening

Hospital/Health experience - highly desirable
Neurological experience
