Our Opportunity:

As a Support at Home provider, Bayside City Council have opportunities for people experienced in aged care service delivery coordination to help us build a service that keeps client needs at the centre of what we do. We optimise client wellbeing while working within the scope of the Support at Home program.

The Care Partner is responsible for coordinating care for Support at Home clients to support them to live at home as safely and independently as practicable. We take pride in setting our

care partners up for success with manageable caseloads that support close working relationships with clients and enabling a focus on proactive care. 

You will join an aged care team committed and acknowledged for achieving exceptional outcomes for clients.

To be successful in this role you will need: (Essential - Key Selection Criteria):

  • A qualification in a health, allied health, social science or community services discipline is desirable
  • Experience in care management/care coordination
  • Demonstrated understanding of the needs and challenges facing people who are ageing and wishing to remain at home within their community
  • Experience in delivery of person-centred care and holistic care management
  • An ability to identify client needs and respond appropriately in an empathetic, objective, respectful and professional manner
  • Well-developed written, verbal, conflict and problem resolution skills

Employment Requirements

  • Satisfactory Criminal History Check
  • Working with Children Check
  • Current Victorian Drivers Licence
