About the Role:

Youโ€™ll work alongside NDIS participants, families, and providers to bring their plans to life. This includes:

  • Supporting participants to build capacity and navigate the NDIS system
  • Connecting clients with services that match their needs and goals
  • Building and maintaining strong relationships with participants and providers
  • Monitoring progress and updating reports/documentation as needed
  • Collaborating with internal and external stakeholders to drive positive outcomes

About You โ€“ Skills & Attributes:

  • Solid administration and documentation skills
  • Strong understanding of mental health and its impact on daily life
  • Collaborating with families, service providers, and other stakeholders
  • Good assessment and observational abilities

Essential Criteria:

  • Qualification in Mental Health, Community Services, Disability or a related field
  • Minimum 3 yearsโ€™ experience in Support Coordination or similar role
  • Experience supporting individuals with physical, intellectual, or psychosocial disabilities

Additional Requirements:

  • Willingness to travel locally as needed
  • Safe, reliable vehicle with comprehensive insurance
  • Ability to work from home in a suitable space
  • Working With Children Check
  • Police Check (or willingness to obtain)
  • NDIS Worker Screening Check
  • Completion of the NDIS Worker Orientation Module
