About the Role:
Youโll work alongside NDIS participants, families, and providers to bring their plans to life. This includes:
- Supporting participants to build capacity and navigate the NDIS system
- Connecting clients with services that match their needs and goals
- Building and maintaining strong relationships with participants and providers
- Monitoring progress and updating reports/documentation as needed
- Collaborating with internal and external stakeholders to drive positive outcomes

- Strong understanding of mental health and its impact on daily life
- Collaborating with families, service providers, and other stakeholders
- Good assessment and observational abilities
Essential Criteria:
- Qualification in Mental Health, Community Services, Disability or a related field
- Minimum 3 yearsโ experience in Support Coordination or similar role
- Experience supporting individuals with physical, intellectual, or psychosocial disabilities
Additional Requirements:
- Willingness to travel locally as needed
- Safe, reliable vehicle with comprehensive insurance
- Ability to work from home in a suitable space
- Working With Children Check
- Police Check (or willingness to obtain)
- NDIS Worker Screening Check
- Completion of the NDIS Worker Orientation Module
