Support Coordinator

About the role
We are seeking a dedicated and experienced Support Coordinator to join our team in Morayfield. This full-time role requires you to be in the office for the first 3 months for training. Following this period, a hybrid work model will be discussed.

As a Support Coordinator you will work closely with clients to effectively utilise their NDIS-funded supports, connect them with suitable service providers and allied health professionals, and monitor their progress towards achieving their plan goals.

Responsibilities include:

• Provide support to clients and their families in accessing and coordinating services.
• Build collaborative relationships with internal and external stakeholders.
• Facilitate the implementation of all supports outlined in NDIS plans.
• Enhance clients skills to coordinate support and engage actively in the community
• Empower clients with a deeper understanding of their plans and how to implement them
• Be available and responsive to client needs, especially during crises.
• Conduct regular progress reviews of client plans, ensuring timely and accurate reporting to the NDIS.

Skills, Qualifications and Experience:

• Excellent interpersonal and communication skills
• Strong written communication skills
• Demonstratable problem-solving skills
• Ability to collaborate effectively
• Understanding or knowledge of the NDIS
• Experience in support coordination or similar
• Familiarity with psychosocial disabilities (advantage)
• Relevant qualifications in disability services, social work, or related fields (desirable)

Requirements
The successful applicant will have or be willing to obtain

• Driver’s Licence
• Blue card/Working with Children Check
• NDIS Worker Screening Clearance (yellow card)
