About the role
This is a part-time Support Coordinator role based in Gold Coast working for Infinity Support For You Pty Ltd, a leader in aged and disability services. As a Support Coordinator, you will play a key part in helping our clients to achieve their goals and live with greater independence and dignity.
What you'll be doing
- Developing and implementing customised support plans for clients based on their individual needs and goals
- Coordinating a range of support services including personal care, domestic assistance, community access and allied health
- Building and maintaining positive relationships with clients, their families and a network of service providers
- Regularly monitoring and reviewing client progress to ensure they are receiving the best possible support
- Advocating for clients and liaising with government agencies and other stakeholders on their behalf
- Maintaining accurate records and reporting on client outcomes
What we're looking for
- Relevant tertiary qualification in a community services or health-related field
- 2+ years' experience working as a Support Coordinator or similar role in the aged care or disability sector
- Strong understanding of the NDIS and ability to navigate the scheme to achieve the best outcomes for clients
- Excellent communication and interpersonal skills, with the ability to empathise and build trust with clients
- Problem-solving mindset and adaptability to respond to changing client needs
- Strong organisational and time management skills
- Compassion, patience and a genuine commitment to making a difference in people's lives
