Role Overview

We are seeking a proactive and compassionate Support Coordinator to help NDIS participants navigate their plans and connect with the right services. This is a dual role that includes some psychosocial recovery coaching responsibilities.

In this position, you will work alongside participants to understand their goals, coordinate services, and build their confidence in managing their own supports over time. This involves liaising with providers, troubleshooting barriers, and ensuring plans are implemented effectively. You will also support participants to develop capacity and independence through a recovery-oriented, trauma-informed approach.

Working collaboratively within a multidisciplinary team, you will advocate strongly for participants and help them overcome complex system challenges. Key Responsibilities

  • Coordinate and implement NDIS supports, ensuring services align with participant goals, budgets, and plan requirements while promoting choice and control.
  • Connect participants with appropriate providers, negotiate service agreements, and maintain strong collaborative relationships with stakeholders, families, and clinicians.
  • Monitor progress and adjust supports proactively, identifying barriers, advocating for participant needs, and preparing evidence for plan reviews.
  • Support participants to build capacity, independence, and confidence in managing their own supports over time using a recovery-oriented, trauma-informed approach.
  • Maintain high-quality documentation, including timely case notes, risk assessments, reports, and compliance with NDIS Practice Standards and company policies.
  • Respond to crises and emerging needs using sound judgement, escalating concerns appropriately to ensure participant safety and stability.
  • Promote participant empowerment, ensuring all interactions uphold dignity, cultural safety, and person-centred practice.

Key Requirements:

  • Experience: Previous experience in support coordination, disability support, case management, or a related field is required.
  • Knowledge: Strong understanding of the NDIS framework, policies, and procedures.
  • Skills: Excellent communication, interpersonal, and organizational skills.
  • Empathy: A genuine passion for working with individuals with disabilities and a commitment to improving their quality of life.
  • Qualifications: Relevant qualifications in social work, community services, or a related discipline are advantageous.
  • Technology: Proficiency with online systems
  • Work Rights: Australian work rights are required for this position.
