ConnectAbility is offering an exciting opportunity for a dedicated and passionate Support Coordinator to join our team on a part-time (50 hours per fortnight) basis. On offer is an 18-month fixed term contract, covering parental leave. About the role
This pivotal role will see you liaise with a range of internal and external stakeholders to allow each customer to maximise their NDIS plan and achieve their individual goals, build capacity and increase resilience for each of our customers. The ideal candidate will also bring an open and creative mindset to the position. In this role you will:
- Manage a portfolio of customers, completing agreed activities within the hours and funded budget articulated in customers’ NDIS Plan
- Assist customers to connect to, engage and build capacity with informal and mainstream funded supports and services as articulated in their NDIS Plan and which assist them achieve their goals
- Build and sustain relationships with external service providers and mainstream supports for the purpose of maximising opportunity for customers.
- Build capacity in customers and/or their family to maintain support relationships with the end goal being a reduction in the customers need for support coordination into the future
This position will commence from 26 May 2026 and will conclude on 31 December 2027. This is an onsite role, with flexibility for the successful applicant to operate from our Warabrook, Kurri Kurri or Erina office.
About you
- Tertiary qualifications in social sciences, health or other relevant professional field or a diploma in Disability; with at least three years experience working with people living with a disability or in case management.
- Effectively utilise NDIS packages by using their knowledge of the NDIS, the service system, being assertive, and ‘making things happen’.
- Experience working with customers and their families in times of crisis
- Excellent verbal and written communication skills.
- Excellent time management, administration with the ability to handle multiple conflicting priorities.
- Ability to maintain accurate, professionally appropriate and current records and written reports.
- Experience working with people living with a disability who may require additional support with their communication or cultural needs.
- Demonstrated experience with facilitating multi-disciplinary coordinated planning where multiple providers work together to wrap-around services for people with disabilities and develop collaborative relationships and networks between agencies.
- Ability to use Microsoft Office Suite and client information management systems.
- Current Drivers’ License and reliable, comprehensively insured vehicle

- NDIS Worker Orientation Module Completion Certificate
