About the role

We are seeking a Support Coordinator to join our team at Recognise Support Services in Busselton, WA. This full-time role is responsible for coordinating and overseeing the delivery of disability support services to our clients in the local community.

What you'll be doing

  • Assess client needs and develop tailored support plans to meet their goals and requirements
  • Coordinate and manage the delivery of support services to our clients
  • Monitor client progress and make adjustments to support plans as needed
  • Liaise with clients, their families, and other Service Providers to ensure seamless care
  • Maintain detailed client records and prepare reports as required
  • Contribute to the continuous improvement of our disability support services

What we're looking for

  • Relevant experience and/or qualifications in disability services, social work, or a related field
  • Minimum 2 years' experience in a similar support coordination or disability related role
  • Good understanding of the disability support sector and NDIS Legislation
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and their families
  • Strong organisational and time management skills, with the ability to prioritise and multitask
  • Demonstrated problem-solving and decision-making abilities
  • Empathy, patience, and a genuine commitment to improving the lives of our clients
