We are seeking a motivated and experienced Support Coordinator to join our Townsville team. This is a rewarding opportunity to work closely with NDIS participants and play a key role in supporting them to achieve their goals.

About the Role

In this role, you will:

• Support participants to understand and implement their NDIS plans
• Connect participants with appropriate supports, services, and community resources
• Coordinate multiple providers to ensure effective and seamless service delivery
• Build participants’ capacity to manage and direct their own supports
• Monitor progress, address barriers, and adapt supports as needs change

About You

You will bring:

• Experience in Support Coordination, Case Management, or the Community Services sector
• A sound understanding of the NDIS and its processes
• Strong communication and interpersonal skills
• A proactive and solution-focused approach
• A genuine commitment to participant outcomes and advocacy

Requirements

• Current Australian Driver’s Licence and reliable, insured vehicle
• Current Blue Card and NDIS Worker Screening Check (or willingness to obtain)
• National Police Check
• First Aid and CPR certification (or willingness to obtain)
