We are seeking a motivated and experienced Support Coordinator to join our Townsville team. This is a rewarding opportunity to work closely with NDIS participants and play a key role in supporting them to achieve their goals.
About the Role
In this role, you will:
- Support participants to understand and implement their NDIS plans
- Connect participants with appropriate supports, services, and community resources
- Coordinate multiple providers to ensure effective and seamless service delivery
- Build participantsβ capacity to manage and direct their own supports
- Monitor progress, address barriers, and adapt supports as needs change
About You
You will bring:
- Experience in Support Coordination, Case Management, or the Community Services sector
- A sound understanding of the NDIS and its processes
- Strong communication and interpersonal skills
- A proactive and solution-focused approach
- A genuine commitment to participant outcomes and advocacy

Requirements
- Current Australian Driverβs Licence and reliable, insured vehicle
- Current Blue Card and NDIS Worker Screening Check (or willingness to obtain)
- National Police Check
- First Aid and CPR certification (or willingness to obtain)
