About the role
This part-time Support Coordinator role with Expanding Influences is based in Caringbah NSW 2229. As a Support Coordinator, you will play a key part in helping individuals with disabilities and their families access the services and support they need to live independently and participate fully in their communities.
What you'll be doing
- Work closely with participants to understand their goals, needs and preferences and develop tailored support plans
- Coordinate the delivery of supports and services from a range of providers to meet participant needs
- Advocate for participants to ensure they receive the appropriate level of funding and supports
- Monitor and review participant progress, adjusting support plans as required
- Build and maintain relationships with service providers, government agencies and the local community
- Provide guidance and information to participants and their families
- Contribute to the continuous improvement of support coordination practices
What we're looking for
- Relevant tertiary qualification in a human services field such as social work, disability or community services
- Minimum 2 years' experience in a similar support coordination or case management role
- Strong understanding of the NDIS and disability support services landscape
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse clients
- Demonstrated problem-solving skills and ability to think creatively to overcome challenges
- Strong organisational and time management skills with the ability to prioritise tasks
- Empathetic, person-centred approach with a commitment to client choice and control
