About the Role
Aussie Angels Nursing and Disability Services is seeking a motivated and compassionate Support Coordinator to assist NDIS participants in understanding and implementing their plans. In this role, you will empower individuals to build capacity, connect with services, and achieve their goals while promoting independence and inclusion in the community.
Key Responsibilities
- Support participants to understand and effectively use their NDIS plans
- Assist clients to pursue their goals through tailored support coordination
- Connect participants with NDIS providers, community, mainstream, and government services
- Coordinate a mix of supports to meet individual needs and enhance independence
- Build participants’ confidence and capacity to manage their supports
- Monitor service delivery and ensure supports are working effectively
- Assist participants in navigating complex support environments when required
- Maintain accurate documentation and ensure compliance with NDIS standards
Levels of Support Coordination
- Support Connection: Assist participants to connect with informal and community supports
- Support Coordination: Help participants understand and manage their plan, services, and supports
- Specialist Support Coordination: Provide higher-level support for participants with complex needs, ensuring consistent and effective service delivery
Skills & Experience
- Experience in NDIS, disability, or community services
- Strong understanding of the NDIS framework and supports
- Excellent communication and interpersonal skills
- Ability to work independently and manage competing priorities
- Strong problem-solving and organisational skills
- Commitment to person-centred and capacity-building approaches
