ABOUT THE ROLE:
The role of a Support Coordinator & Psychosocial Recovery Coach is to provide individualised support to participants (who have a disability and/or mental illness) of the National Disability Insurance Scheme (NDIS). The Support Coordinator & Psychosocial Recovery Coach will work with the participant to implement the supports within the person's plan for the purpose of building capacity, managing the impact of their disability, and improving their health, well-being, and participation within the community.
Experience as a Support Coordinator/ Psychosocial Recovery Coach is essential.
This role is a Permanent Full Time Role and scheduled to begin their employment in July 2025.
The role is combine working from our office (Hawthorn) and working from home with your clients based in the North/West Melbourne region.
KEY RESPONSIBILITIES:
Conduct assessments to understand clientsβ psychosocial needs, strengths, and goals.
Develop and monitor personalised recovery and NDIS plans, including plan reviews and tribunal supports.
Provide empathetic coaching, emotional support, and skill-building to enhance clientsβ resilience and life skills.
Coordinate services and connect clients with community, housing, employment, and mainstream or specialised supports.
Advocate for clientsβ rights and empower them in their recovery journey.
Maintain accurate client records, progress reports, and documentation.
Engage in ongoing professional development and meet KPI targets efficiently.
REQUIREMENTS:
Minimum Diploma in Community Services, Mental Health, AOD, or Youth Work, or higher qualifications.
At least 1 year of experience in Support Coordination and/or Psychosocial Recovery Coaching.
Strong interpersonal, communication, and documentation skills.
Knowledge of community resources and an ethical, culturally sensitive approach.
Right to work in Australia
Satisfactory completion of safety screening including a National Police check, International Police check (if required), a NDIS Working screen check
A current Victorian Working with Children Check (Employee)
Current Victorian Drivers License
