About the Role

As a Support Worker, you will provide person-centred care to clients in their homes and communities.

Key Responsibilities

  • Provide social & community support (outings, activities, and community programs)
  • Offer emotional support and companionship to enhance client well-being
  • Assist with in-home support and daily living tasks (personal care, meal preparation, and household duties)
  • Support medication management and health monitoring to ensure client safety
  • Advocate for clients and assist them in accessing relevant services and resources
  • Maintain accurate documentation and reporting in line with organisational and NDIS requirements
  • Perform other duties as required, based on individual client needs and care plans

About You

  • Compassionate, empathetic & client-focused
  • Reliable, flexible & able to work independently
  • Strong communication and interpersonal skills
  • Organized & able to support complex needs
  • Comfortable using technology for reporting

Requirements

  • Cert III in Individual Support / Disability / Ageing (or higher)
  • Current First Aid, CPR & Manual Handling
  • Current Police Check
  • NDIS Worker Check (NDISWC)
  • Working With Children Check (WWCC)
  • Valid ID & Visa (if applicable)
  • Completion of NDIS E-learning Modules
  • Infection Control Training
  • Medication & Diabetes Training
  • Valid Driver's Licence & Reliable Vehicle
  • Valid work visa or other legal work authorisation in Australia
