Part-Time Team Leader (Disability Support Worker)

Swan Home Care Services is a growing NDIS registered provider delivering high-quality, person-centred disability support across Perth and surrounding areas. Due to continued growth, we are seeking two experienced Part-Time Team Leaders (Support Workers) to join our leadership team.

This is a hands-on leadership role, combining direct support shifts within the teams you oversee, along with allocated administrative and leadership hours.


About the Role

As a Team Leader, you will play a key role in ensuring safe, consistent and high-quality service delivery across multiple support settings. You will provide on-the-floor leadership, mentor support workers, and act as a point of contact between participants, families and management.

Availability is required Monday to Friday for AM and PM shifts, with flexibility to travel between different team settings.


Key Responsibilities

  • Provide leadership and support to disability support workers within allocated teams
  • Deliver direct supports to participants as required
  • Support implementation of Behaviour Support Plans and Positive Behaviour Support strategies
  • Monitor and maintain high standards of service delivery and compliance
  • Assist with staff supervision, mentoring, and onboarding
  • Liaise with management regarding incidents, concerns, and service improvements
  • Complete administrative duties, including documentation, shift notes, and reporting
  • Promote a positive, professional, and values-driven team culture

Essential Criteria

  • Demonstrated experience as a Team Leader or Senior Disability Support Worker
  • Strong experience in behaviour support and complex support needs
  • Proven ability to deliver high-quality, person-centred services
  • Excellent communication, leadership, and problem-solving skills
  • Ability to work independently and as part of a leadership team
  • Availability Monday to Friday – AM and PM shifts
  • Willingness and ability to travel between multiple service locations

Requirements

  • Certificate III or IV in Disability, Community Services or equivalent
  • NDIS Worker Screening Check
  • Working With Children Check
  • First Aid & CPR (current)
  • Medication competency (or willingness to obtain)
  • Current Driver’s License and reliable vehicle
  • Ability to complete required documentation and use digital systems
