We are currently seeking a compassionate and experienced Aged Care Case Manager to join our growing team. This role offers the opportunity to support older Australians by coordinating tailored care plans, connecting them with essential services, and ensuring their needs and goals are met, while contributing to our mission of empowering people to live with dignity and independence.

Responsibilities:


Conduct comprehensive assessments of clients’ needs, strengths, and goals in line with My Aged Care guidelines.

Develop, implement, and monitor individualised care plans, ensuring they are person-centred and goal-focused.
Provide advocacy, information, and referral services to clients and their families/carers regarding aged care supports and community resources.
Coordinate and liaise with service providers, health professionals, and community organisations to ensure seamless delivery of care.
Monitor client progress, review services regularly, and adjust care plans as required.
Understanding of the Aged Care Quality Standards and person-centred care principles.
Strong communication, negotiation, and interpersonal skills, with the ability to build positive relationships with clients, families, and stakeholders.
Proven ability to manage a caseload, prioritise tasks, and work independently as well as part of a team.
High level of accuracy in documentation, care planning, and record-keeping.
Proficiency in Microsoft Office and client management systems.
Ensure compliance with legislative requirements, quality standards, and organisational policies.
Maintain accurate, up-to-date case notes, care plans, and documentation in line with confidentiality and privacy requirements.
Support clients in navigating My Aged Care systems, including assessments, referrals, and accessing appropriate funding streams.
Promote client independence, dignity, and quality of life while respecting cultural, social, and individual preferences.
Participate in team meetings, training, supervision, and continuous improvement activities.
Account Management
Develop and implement continuous improvement initiatives
Actively contribute to the business in a positive manner
All other duties as directed

Essential Criteria:


Relevant tertiary qualifications in Social Work, Community Services, Nursing, Allied Health, or a related field.
Demonstrated experience in case management within aged care, health, or community services.
Current Australian Driver’s License and access to a reliable vehicle.
High level of accuracy in documentation, care planning and record keeping.
Understanding of the aged care quality standards and person centred care principles.
Current First Aid and CPR Certification.
NDIS Screening Check.
Police Check.
Understanding of risk assessment, safeguarding, and duty of care requirements.

Preferred Criteria:


Bilingual
Proven experience in the My Aged Care sector.
Demonstrated experience with researching and organizing quotes and report writing.
