Join BASScare as a Community Services Intake Officer and play a key role in helping older adults access the support they need to remain independent, connected, and well.

About the Role:


As the first point of contact for many new clients and families, you’ll guide people through their aged care journey. You’ll conduct assessments, develop tailored care plans, and coordinate services across a range of programs, ensuring each person receives the support that best meets their needs.

Key Responsibilities:


Conduct client intakes and assessments (in-home, over the phone, or in-centre).
Develop, review and update individual care plans in partnership with clients, carers, and health professionals.
Monitor changing client needs and ensure services remain responsive and appropriate.
Maintain accurate, high-quality records and support compliance and audit processes.
Build strong relationships with clients, internal teams, and community stakeholders.
Contribute to service improvements and quality assurance activities.

What You’ll Bring:


Experience with client intake, assessment, and care planning.
Solid understanding of consumer-directed care principles.
Strong admin, IT and organisational skills.
Excellent communication, interpersonal, and problem-solving skills.
Comfortable working independently and collaboratively.
Committed to inclusion, dignity, and person-centred practice.
Police Check (current) and valid Driver’s Licence.

Desirable but not essential:


Familiar with government-funded aged care programs like My Aged Care, CHSP, or Support at Home.
