About the role:
The Registered Nurse plays a vital role in delivering high-quality nursing care, conducting health assessments, and ensuring the well-being of participants at RISU Care. This role involves collaboration with healthcare professionals, support teams, and participants to provide person-centered care while maintaining compliance with NDIS and regulatory standards.
Available 2 days per week: 9am till 5pm
Duties will include:
Compliance & Documentation
Follow RISU Care’s policies, procedures, and management instructions.
Ensure all documentation and reporting comply with NDIS and regulatory standards.
Keep participant information up to date on OneDrive and Brevity, including care plans and service agreements.
Monitor daily progress notes to identify any changes or concerns in participants' health status, follow up with staff as necessary, and update care plans accordingly.
Review incidents, hazards, complaints, feedback, and compliments data, taking appropriate actions to mitigate risks or improve practices.
Provide daily report to the clinical team via an email.
Clinical Care & Participant Well-being
Deliver comprehensive nursing assessments, care planning, implementation, and evaluation for participants.
Provide clinical care and treatments as prescribed, including but not limited to regular health checks, wound management, medication administration, and monitoring vital signs.
Identify changes in participants’ health status, follow up with staff as necessary, and adjust care plans accordingly.
Collaborate with healthcare professionals to develop and implement individualized care plans tailored to participants' needs.
Provide education and support to participants and their families regarding health conditions, medications, and self-management strategies.
Collaboration & Continuous Improvement
Work closely with the Client Services Manager and support team to ensure person-centered active support is consistently delivered to participants.
Collaborate with the Client Services Manager to conduct risk assessments and develop individualized support plans for both new and existing participants.
Participate in team meetings and contribute to the continuous improvement of service delivery.
Be open to new ways of enhancing participants' quality of life and respond to challenges with innovative solutions.
The successful candidate will have:
A degree in Disability, Social Service, Nursing or recognised equivalent.
If a Registered Nurse, then current registration with AHPRA and evidence of continued professional development in accordance with the AHPRA standards.
NDIS workers screening check clearance.
Working with Children Check
Current Driver’s license.
Experience:
Recent experience in the disability, aged, community or health sectors
Experience in development and review of health support plans or other care plans.
Experience supporting People with Disability who have complex and high support medical needs.
Knowledge of continuous quality improvement principles
An understanding of ‘human rights informed’ quality management principles and standards
An understanding of the NDIS key principles
Knowledge of the interface between quality, practice and safeguards in the prevention of abuse and neglect of people with disability
