Aged Care Support Worker, Community Care Assistant, Disability Support

About the role:

Australia Disability Services is seeking an experienced Disability Support Worker to join our team in Point Cook, VIC on a contract basis. As a Disability Support Worker, you will play a vital role in supporting individuals with a range of disabilities to live independently and participate actively in their communities. This is a rewarding opportunity to make a meaningful difference in the lives of our clients.

What you'll be doing:


Provide personalised, one-on-one support to clients with a range of physical, intellectual, and cognitive disabilities.
Assist clients with daily living activities such as personal care, meal preparation, medication management, and household tasks.
Facilitate social and recreational activities to promote clients' independence, community engagement, and overall well-being.
Advocate for clients' rights and ensure their needs are met with dignity, respect, and empathy.
Collaborate with multidisciplinary teams to develop and implement individualised support plans.
Maintain detailed client records and documentation in accordance with organisational policies and procedures.

What we're looking for:


A Certificate III or IV in Individual Support (Disability) or equivalent qualification.
Demonstrated experience in providing direct personal care and support to individuals with a range of disabilities.
Strong communication and interpersonal skills, with the ability to build rapport and work effectively with clients, their families, and multidisciplinary teams.
Excellent problem-solving, decision-making, and organisational skills.
Compassionate, patient, and person-centred approach to care.
A valid driver's licence and access to a reliable vehicle.
Current First Aid and CPR certification.
NDIS Worker Screening Check.
