About the Role
We are seeking an experienced and well-connected Hospital & Referral Partnerships Manager to drive strategic growth by building strong, trusted relationships with key hospital discharge planners, NDIS Support Coordinators, Local Area Coordinators (LACs), clinicians, and referrers.
This is a senior business development and stakeholder engagement role, focused on increasing referrals for NDIS-funded supports including Short-Term Accommodation (STA), Medium-Term Accommodation (MTA), Supported Independent Living (SIL), SDA and Community Nursing.
You will act as the key point of contact for health sector and NDIS referral sources, developing long-term partnerships and rapid response pathways for hospital discharge and crisis placements.
Key Responsibilities:
Build, manage, and grow strategic referral relationships with:
Hospital discharge teams
Social workers
Allied health professionals
NDIS Planners, LACs, and Support Coordinators
Promote and represent the organisationโs range of services (SIL, MTA, STA, Community Nursing, Respite)
Lead hospital engagement efforts to support timely and coordinated discharges into supported accommodation
Identify referral trends and high-demand locations to inform service development and accommodation planning
Collaborate closely with internal intake, clinical, and operations teams to ensure referral-to-placement success
Represent the organisation at hospital network meetings, NDIS expos, and sector forums
Maintain a CRM of referral relationships and pipeline activity
Key Selection Criteria:
Essential
Minimum 3+ yearsโ experience in a hospital liaison, referral coordination, or NDIS stakeholder engagement role
Strong existing networks within hospital systems, discharge planning teams, and/or NDIS referral networks
Proven ability to generate high-value referrals and maintain professional relationships across clinical and community settings
Knowledge of NDIS-funded supports, particularly SIL, STA, MTA, SDA and Community Nursing
Excellent verbal communication and interpersonal skills
Highly organised with CRM or pipeline tracking experience
Current driverโs licence and willingness to travel to referral sites
Desirable
Clinical, allied health, case management or care coordination background
Understanding of hospital discharge barriers and NDIS access processes
Previous experience in a similar role within a disability or health organisation
