About the role

The NDIS Care Coordinator is responsible for supporting individuals to achieve their goals and aspirations by developing and coordinating care plans that enhance independence and quality of life. This role ensures that customers receive the appropriate services, funding, and support they require.

Key responsibilities include:

  • Developing and managing customer care plans that align with individual goals and needs.
  • Coordinating services to ensure customers receive the right level of support.
  • Monitoring and reviewing plans to adapt to changing needs and circumstances.
  • Liaising with key stakeholders, including families, support coordinators, plan managers, and healthcare professionals.
  • Ensuring compliance with NDIS and Aged Care requirements, including budget management and reporting.
  • Identifying opportunities for social and community engagement to promote inclusion and wellbeing.
  • Managing incidents and feedback, ensuring timely resolutions and continuous improvement.
  • Maintaining industry knowledge and staying up to date with sector developments.

This role is ideal for someone who enjoys working directly with individuals to help them navigate support systems and make informed choices about their care.

What We Are Looking For

  • Minimum Certificate III in Individual Support, Disability, or Aged Care (or equivalent).
  • Experience in support coordination, case management, or a related field.
  • Strong understanding of NDIS, Aged Care, and community support services.
  • Excellent communication and relationship-building skills.
  • Strong organisational and problem-solving abilities.
  • Ability to work both independently and collaboratively as part of a team.
  • Proficiency in Microsoft Office and business information systems.

Essential:

  • Working with Children (Blue Card).
  • Working with Adults with Disability Card (Yellow Card) or NDIS Workers Screening Card/Check.
  • Current Class C Driver's License.
  • Current NDIS Workers Module.
